Opening opportunities for individuals with disability
Disability services are offered to arm members with disabilities with the tools to address their needs and live more independent lives, allowing them to contribute to their community. Our disability services include the following:
Day Program
Our FCCS Day Program supports individuals with developmental and intellectual disabilities through day habilitation, vocational training, and employment assistance.

We provide a range of activities tailored to their interests and abilities. This includes planting, yoga, music therapy, arts and crafts, social skills classes, and pet therapy. Our goal is to promote independence and skill development while fostering a nurturing environment.
Supported Employment
Our Supported Employment Program can be provided virtually or in person, with or without the involvement of the person's employer. This service helps members explore their career interests, abilities, and potential, while receiving necessary support to enhance their skills during the hiring process.
Our supported employment services at Four Corners Community Services encompass:
- Systematic Instruction and Training
- Job Coaching
- Benefit Support
- Job Development and placement
- Negotiation with Prospective Employers
- This service provides travel training, planning, and workplace support to enable successful integration into the job setting, including services not directly related to job-skill training.
Community-Based Support
Our Community-Based Support Program provides assistance to members whether they live in or out of their home. The goal of this program is to achieve or maintain our members’ community inclusion, promote independence, and improve productivity and family functioning as outlined in their service plan. This program includes assistance with community-based activities, training, and supervision of individuals as they learn and perform the various tasks required for basic self-care, social skills, and daily living activities.
